The opportunity

Job Description

Midland Care Connection is seeking a Nurse Practitioner to provide comprehensive, patient-centered primary care and hospice/palliative care services to older adults and individuals in rehabilitation centers, skilled nursing facilities, assisted living communities, and independent living settings.

This role offers the opportunity to work across two clinical areas: APRN Physician Services, where you will assess and manage acute and chronic health conditions in residential and long-term care settings, and Hospice & Palliative Care, where you will focus on comfort, symptom management, and quality of life for patients with serious or life-limiting illnesses.

Responsibilities

  • Perform comprehensive physical assessments and evaluate patients with acute and chronic health conditions
  • Order and interpret diagnostic tests
  • Manage medications and prescribe treatments
  • Develop and update individualized treatment plans based on patient needs and goals of care
  • Address acute illnesses, chronic disease management, dementia care, and geriatric conditions
  • Manage complex symptoms, pain, and end-of-life care concerns
  • Discuss goals of care and treatment options with patients and families
  • Collaborate with physicians, nurses, caregivers, families, hospice, home health, rehabilitation teams, and other healthcare professionals to ensure coordinated and continuous care
  • Work with interdisciplinary teams including social workers, chaplains, and other care professionals
  • Maintain accurate clinical documentation
  • Help reduce unnecessary hospital visits by providing accessible care within residential and long-term care settings

Required Qualifications

  • Current, unrestricted Nurse Practitioner/APRN license in Kansas
  • Current national board certification as a Nurse Practitioner through an approved certifying body
  • Graduate degree from an accredited Nurse Practitioner program
  • Current DEA registration and ability to prescribe medications
  • Current BLS certification
  • Experience providing direct patient care as a Nurse Practitioner
  • Strong assessment, clinical decision-making, and communication skills
  • Ability to work collaboratively as part of an interdisciplinary care team
  • Ability to provide compassionate, patient-centered care to individuals with serious or life-limiting illnesses
  • Ability to work independently and manage multiple patient needs while maintaining accurate clinical documentation
