The opportunity

Job Description

Galileo is seeking a Nurse Practitioner to join our Population Health team, providing remote telephonic and video-based care to patients in our Transitions of Care program. In this role, you will deliver comprehensive follow-up care to patients who have recently been hospitalized and discharged, working to improve health outcomes and support complex patient populations across the United States.

Clinical Setting

Galileo operates as a 24/7 digital-first medical group delivering high-quality, comprehensive primary care and transition of care follow-up services. You will work remotely, providing care via phone and video to diverse and complex patients, including those with Medicare and Medicaid coverage.

Responsibilities

  • Conduct telephonic outreach and scheduled video/phone visits for recently hospitalized and discharged patients
  • Perform medication reconciliation and symptom screening
  • Order follow-up labs, medications, and supplies as needed
  • Build trust with patients through meaningful conversations and explain Galileo's care model
  • Connect patients to appropriate services and support
  • Coordinate care across settings including hospitals, emergency departments, pharmacies, and community providers
  • Collaborate with the nursing team on patient care
  • Meet individual performance targets while contributing to team-level population health goals

Schedule

Galileo operates in 4-hour shifts covering mornings, afternoons, evenings, and overnight hours on weekdays and weekends. You are required to work a minimum of 24 hours per week. At 24 or more hours per week, you will be considered full-time and benefits eligible. Providers are expected to work weekends on a monthly basis; specific scheduling will be influenced by total hours committed and patient care demands.

Required Qualifications

  • Certified Nurse Practitioner in Adult, Family, or Geriatrics
  • Active Illinois state licensure
  • Ability to obtain additional state licensures
  • Minimum 5 years of post-graduate clinical experience in Primary Care or Geriatrics (2 of these years may be substituted with experience in a Skilled-Nursing facility or Long Term Care facility)
  • Direct experience engaging Medicare and Medicaid populations managing complex, comorbid conditions
  • Comfort in a digital-first environment with experience in or genuine appetite for virtual care delivery
  • Solid technology proficiency
  • Excellent written and verbal communication skills

Preferred Qualifications

Compensation

  • Regular rate: $75 per hour
  • Premium rate: $85 per hour
