The opportunity

Job Description

Coastal Hospice is seeking a Nurse Practitioner (PRN) to provide advanced clinical care to patients with terminal illnesses. As a vital member of the hospice care team, you will assess patient needs, develop and implement individualized care plans, manage symptoms, and support patients and families through the end-of-life process. This role works collaboratively with an interdisciplinary team including physicians, nurses, social workers, chaplains, and other hospice staff.

Responsibilities

  • Conduct comprehensive assessments of patients' physical, emotional, and psychosocial needs
  • Develop, implement, and periodically review individualized care plans in collaboration with the interdisciplinary team
  • Provide expert management of pain and other distressing symptoms
  • Prescribe medications and treatments in accordance with state and federal regulations
  • Educate patients and families about disease processes, symptom management, and hospice philosophy
  • Provide guidance on advance directives and end-of-life decision-making
  • Participate in interdisciplinary team meetings to discuss and update patient care plans
  • Maintain accurate and timely documentation of patient assessments, care plans, and treatments
  • Ensure compliance with hospice policies, state and federal regulations, and accreditation standards
  • Participate in quality assessment and performance improvement (QAPI) initiatives
  • Contribute to the development and implementation of evidence-based practices in hospice care
  • Provide support and interventions during patient crises, including transitions to higher levels of care or emergency situations
  • Mentor and provide education to nursing staff and other team members as needed
  • Participate actively in continuing education and in-services
  • Perform other duties as assigned

Required Qualifications

  • Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
  • Current state licensure to practice as a Nurse Practitioner
  • Valid driver's license and proof of current auto liability insurance
  • No listing in the OIG Excluded Provider listing

Preferred Qualifications

  • Three years of hospice or palliative care experience

  • Knowledge of age-specific patient needs appropriate for this position
  • Ability to maintain confidentiality of patient information and business practices
  • Keeps abreast of changes in healthcare law
  • Cooperates fully in all risk management activities and investigations
