Telecare is seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide direct psychiatric care and medication management to individuals served through the King County Program of Assertive Community Treatment (PACT). As a senior member of the clinical team, you will work collaboratively with a multidisciplinary team to develop and deliver recovery-focused treatment plans.
PACT serves individuals with severe and persistent mental illness who experience difficulties with daily living activities. The PACT team is mobile and delivers services in community locations rather than expecting consumers to come to a program office. Approximately 75% or more of services are provided outside of program offices in locations that are comfortable and convenient for consumers. The team works together to provide the majority of treatment, rehabilitation, and support services consumers need to achieve their goals, including assistance with finding and maintaining residence and employment in community settings.
