Matrix Medical Network is seeking a Nurse Practitioner to conduct comprehensive in-home health assessments for adult and geriatric patients across the nation. During visits lasting up to one hour, you will review patients' medical history, current health status, medication adherence, social environment, and other risk factors to identify care gaps and improve health outcomes. Assessments may take place in patients' homes or skilled nursing facilities.
Responsibilities
- Conduct adult and geriatric health assessments including medical history, diagnosis, treatment review, and health education
- Administer and order point-of-care and lab-based testing as needed
- Provide physician referrals and case management referrals with clear documentation according to Matrix guidelines and protocols
- Collaborate with primary care physicians, case managers, social workers, family members, and other healthcare personnel
- Provide patient education and follow-up coordination with primary care physicians
Clinical Setting
You will work in patients' homes and skilled nursing facilities, meeting members in their own environments to provide personalized care that captures insights difficult to obtain during routine office visits.
Schedule and Travel
- PRN (as-needed) position with flexible scheduling based on your availability
- Assessments conducted Sunday through Saturday, 8 am to 7 pm
- Work day, evening, and/or weekend shifts according to your schedule
- Must be able to travel up to 80-90% of the time for extended periods, including overnight travel and weekends across your assigned state and possibly neighboring states
- Valid state driver's license required for your state of residence
Compensation
- Earn income based on the number of assessments completed
- Per-assessment completion rate: $90-$115 depending on state of residence
- Monthly earnings for those completing 20-100 visits average: $1,800-$11,500 per month
- Increase earning potential by offering greater availability and traveling outside your home location
- Full-time opportunities available for PRN providers, including full-time sign-on bonus

Required Qualifications
- Master's degree or commensurate experience with satisfactory completion of NP licensure
- Current NP licensure in state of practice with prescription authority or ability to obtain prescriptive authority as needed
- AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
- Current BLS, ACLS, or CPR certification
- Minimum one year of NP experience
- Valid state driver's license for state of residence
- Automobile insurance coverage meeting the following minimums for your state of residence (or ability to obtain):
- Bodily Injury: $100,000 per person/$300,000 per incident
- Property Damage: $50,000
- Collision and comprehensive deductibles: $1,000 or less
- Strong computer skills and familiarity with employee health and medical record software
- Excellent verbal and written communication skills
