Heart'n Soul Hospice is seeking a Nurse Practitioner on a PRN basis to join our team providing compassionate end-of-life and palliative care to patients and families facing life-limiting illnesses. This role offers the opportunity to work in a patient-centered environment focused on delivering high-quality, dignified care during critical life transitions.

Clinical Setting

You will provide advanced practice nursing services within a hospice care model, working collaboratively with an interdisciplinary team including attending and consulting physicians, and other hospice personnel.

Responsibilities

  • Consult with and educate patients and families regarding disease process, self-care techniques, and end-of-life care planning
  • Collaborate with the interdisciplinary team in developing and implementing patient and family plans of care to ensure quality outcomes
  • Interact with attending and consulting physicians regarding palliative care decisions in collaboration with the hospice physician
  • Monitor and communicate changes in patient needs and outcomes of interventions to physicians and relevant personnel
  • Evaluate patient and family response to care and adjust approaches accordingly
  • Submit all required documentation in a timely manner
  • Ensure compliance with local, state, and federal laws, Medicare regulations, and agency policies and procedures
  • Perform additional duties as assigned

Required Qualifications

  • Graduate of a Baccalaureate School of Nursing
  • Current state license as a Registered Nurse
  • Current state license as an Advanced Practice Nurse (APN)
  • Current driver's license with reliable transportation
  • Ongoing valid and current auto liability insurance
  • Two years of experience as an APN in a clinical care setting
  • Comprehensive knowledge of pharmacology, palliative care, and end-of-life care

Preferred Qualifications

  • Prior experience in hospice or home health settings
