M Health Fairview is seeking a Site Medical Lead - Nurse Practitioner for the M Health Fairview Clinic - North Branch location. This role combines clinical primary care practice with front-line provider leadership responsibilities. You will serve as the point of contact for addressing provider and patient issues, working in partnership with the Clinic Manager and Clinical Supervisor to meet day-to-day patient care needs and support execution of key projects.

Responsibilities
Clinical Practice
- Perform all clinical practice duties as required within the primary care provider role
- Provide culturally sensitive care adjusted to reflect the developmental level and cultural differences of the population served
- Partner with patients and caregivers in care and decision-making
- Communicate respectfully and ensure a safe, secure environment
- Individualize plans of care to meet patient needs
- Modify clinical interventions based on the population served
- Provide patient education based on assessment of learning needs
Provider Leadership
- Serve as the first point of contact to address front-line provider or patient issues
- Work in partnership with the Clinic Manager and Clinical Supervisor to address issues and escalate to the Medical and/or Operations Director as needed
- Respond first to COMPASS events and report outcomes to the medical director
- Ensure completion of annual provider requirements, including LMS requirements and license renewal
- Serve as champion for execution of Service Line quality and safety improvement projects
- Facilitate monthly staff meetings in collaboration with the clinic management team
- Support recruitment for the clinic in partnership with the Medical Director, including participation in interviews and candidate selection
- Coordinate onboarding and mentoring of new providers, including check-ins at 30/60/90 days and 6/12/18 months
- Assist the Medical Director in the annual performance review process by gathering provider documentation and providing performance feedback
- Complete mid-season reviews on each of the clinic's providers and report findings to the medical director
- Meet and collaborate regularly with the Clinic Leadership Team
Operational Collaboration
- Collaborate with the Clinic Manager, Clinical Supervisor, and SACC to evaluate clinic schedules, drive operational efficiency, and promote a team environment
- Achieve productivity, financial, and operational goals
- Demonstrate ability to provide care adjusting approaches to reflect developmental level and cultural differences
- Foster a culture of improvement, efficiency, and innovative thinking
- Complete all required learning relevant to the role
- Comply with and maintain knowledge of all relevant laws, regulations, policies, procedures, and standards
Employment Type and Schedule
- 1.0 - 0.70 FTE position (0.1 FTE allocated to Site Medical Lead responsibilities)
Required Qualifications
- Graduate of an accredited formal Family Nurse Practitioner training program
- Board certification (BC/BE) through the AANP or ANCC
- Licensed or ability to obtain APRN license with the Minnesota Board of Nursing
- Ability to obtain and/or maintain DEA certification for the State of Minnesota
- Meet threshold criteria and qualifications for Credentialing and Privileges
- Minimum two years of Nurse Practitioner experience required

- Previous medical leadership experience
Compensation
- Annual salary: $147,952 - $162,747 (based on full-time 1.0 FTE)
