Nurse Practitioner-Pre Heart Transplant - Mount Sinai Hospital - Full Time - Day

Mount Sinai Hospital is seeking a Nurse Practitioner to join the Pre Heart Transplant program in a full-time, day shift position. As an independent practitioner, you will diagnose, treat, and prescribe for patients within your specialty area of practice. You will work collaboratively with the interdisciplinary team to provide comprehensive care to patients preparing for heart transplantation.

Clinical Setting

This role is based at Mount Sinai Hospital, one of the largest academic medical systems in the New York metro area. Mount Sinai is consistently ranked by U.S. News & World Report, including top 5 in Cardiology/Heart Surgery.

Responsibilities

• Assess patient health status through comprehensive health history and physical examination
• Initiate and order therapeutic and diagnostic procedures, consultations, and laboratory examinations
• Prescribe medication regimens and interpret findings according to accepted standards of care and practice guidelines
• Alter plan of care based on patient response and clinical assessment
• Educate and counsel patients in health promotion, disease prevention, and management of acute and chronic illnesses
• Document findings and plan of care in the patient's medical record
• Consult with physicians according to established standards, policies, procedures, and protocols
• Initiate referrals to specialty services and foster continuity of care
• Participate in team meetings and conferences to enhance interdisciplinary care delivery
• Maintain patient and employee confidentiality
• Develop individualized plans of care based on diagnoses and patient needs
• Record, report, and interpret patient responses to the plan of care
• Perform patient record reviews with the collaborating physician at least every three months if utilizing a collaborative practice agreement
• Complete Focused Professional Practice Proctoring Evaluation (FPPE) or Ongoing Professional Practice Proctoring Evaluation (OPPE) as required by Medical Staff
• Practice as a member of the allied health staff according to Medical Staff rules and Hospital bylaws
• Serve as a resource person to nurses and other health team members
• Participate in peer review and departmental committees
• Incorporate research findings into clinical practice and participate in approved nursing and medical research studies
• Maintain current knowledge through self-directed learning, literature review, and educational programs

Required Qualifications

• Active New York State RN license with current registration
• Certification as a Nurse Practitioner by the New York State Department of Education with current registration
• Master's degree program designed to prepare an NP to diagnose, treat, and prescribe within specialty area of practice, or successful completion of an approved supplemental education program (post-master's NP certificate)
• Previous progressive experience relevant to pre-heart transplant clinical practice
• National Board Certification in Advanced Practice Nursing (required for all NPs hired effective June 1, 2019; for NPs who bill for professional services, required regardless of hire date; may be deferred up to 1 year if approved by Chief Nursing Officer or designee)
• Credentialing through the Credentialing Committee of the Medical Board with approved Delineation of Privileges
• Drug Enforcement Administration (DEA) certificate
• Basic Life Support (BLS) certification
• Advanced Cardiac Life Support (ACLS) certification
• Ability to perform history taking and physical assessment techniques
• Ability to manage and coordinate a patient caseload
• Physical stamina and manual dexterity to perform a variety of tasks and skills
• Excellent interpersonal and communication skills
• Creative problem-solving and critical thinking abilities
• Leadership skills

Education and Certification Requirements

Nurse Practitioner Certification:

• Graduate of New York State registered Nurse Practitioner Program or equivalent, OR certified as a Nurse Practitioner by a national association via examination
• Completion of a pharmacy component of not less than required semester hours
• Instruction in New York State and Federal Laws relating to prescriptions and recordkeeping (3-hour course)
• Collaborative Practice Agreement with a physician and designated protocols if less than 3,600 hours experience as a practicing Nurse Practitioner, OR Collaborative Relationship with a physician based on employment in clinical practice in the NP role at Mount Sinai Hospital if documented work history as a Nurse Practitioner with greater than 3,600 practice hours (note: all NPs regardless of status will be placed on FPPE initially)

Additional Certifications as Required by Specialty:

• Pediatric Advanced Life Support (PALS)
• Neonatal Advanced Life Support (NALS)
• The Neonatal Resuscitation Program (NRP)

Compensation

• Hourly rate: $84.59
• Actual salaries depend on a variety of factors, including experience, education, and operational need
• Salary range does not include bonuses, incentive pay, differential pay, or other forms of compensation or benefits
