Nurse Practitioner - PPV Covering Macon and Fayetteville

Compassus is seeking a Nurse Practitioner to provide palliative and hospice care covering the Macon and Fayetteville areas. This position offers flexibility in scope, allowing you to focus on palliative care, hospice care, or hospice face-to-face visits based on organizational needs.

In this role, you will care for patients facing chronic, complex, and life-threatening conditions, providing comprehensive assessments, disease and treatment education, and symptom management. You will work collaboratively with primary care physicians, specialists, the interdisciplinary team, and the assigned Compassus Medical Director to ensure patients and families maintain maximum comfort and coping.

Responsibilities

If providing Hospice Care:

• Perform routine and emergency assessments, including recertification visits for patients in their 3rd benefit period or beyond
• Complete routine and emergency assessments as indicated by departmental policy
• Record observations, treatments, and pertinent clinical information
• Communicate findings and symptom management measures with the Medical Director, Attending Physician, and interdisciplinary team
• Provide physical and emotional support to patients and families during crisis periods
• Prevent unnecessary hospitalizations by meeting patient and family needs in the home setting

If providing Palliative Care:

• Perform routine and urgent bedside assessments with patient and family education
• Order, perform, and interpret laboratory and radiology tests within scope of practice
• Prescribe medications, including controlled substances as delegated and licensed
• Order treatments and durable medical equipment as indicated
• Perform therapeutic and corrective measures including urgent care
• Consult with palliative care physicians and collaborate with other physicians as needed
• Assist in all facets of care coordination for palliative care referrals
• Prepare and maintain accurate patient records and documentation to support medical practice and reimbursement
• Complete accurate billing and coding activities according to CMS guidelines
• Assist patients and families with goal setting related to treatment options and advanced care planning
• Participate in monthly interdisciplinary team meetings
• Supervise nurse trainees, including graduate and advanced practice nurses in field placements
• Participate in supervision of medical students, residents, fellows, and other trainees
• Provide training and continuing education for staff
• Assist in development of clinical practice guidelines and standards

If providing Hospice Face-to-Face Visits:

• Complete routine and emergency recertification visits for patients in their 3rd benefit period or beyond
• Record observations, treatments, and pertinent information
• Communicate findings with the Medical Director, Attending Physician, and interdisciplinary team
• Provide physical and emotional support during crisis periods

All Positions:

• Educate patients and caregivers regarding disease process, symptom control, wound care, treatment options, prognosis, and advance care planning
• Maintain confidentiality of patient, family, staff, and organizational information
• Comply with applicable laws and regulations regarding collaborative agreements
• Flex schedule to meet agency needs, including after-hours emergency visits and admissions
• Perform other duties as assigned

Required Qualifications

• Master's degree in Nursing
• Active and unencumbered CNS or Nurse Practitioner license in state of employment
• Certification as a Nurse Practitioner in Adult, Family, Acute, or Geriatric
• Active NPI number
• Maintain active DEA certification if state allows prescription of controlled substances by Advanced Practice Providers
• Current collaborative physician agreement in place (all states except Iowa)

Experience Requirements

If providing Hospice Care:

• Minimum of one year of nursing practice or equivalent experience
• Hospice, oncology, home health, and/or long-term care experience preferred

If providing Palliative Care:

• Minimum of five years of nursing experience
• Clinical nursing care in oncology, hospice, geriatrics, or intensive care preferred

Skills and Abilities

• Strong mathematical skills including ability to add, subtract, multiply, divide, and compute rate, ratio, and percentage
• Ability to read, analyze, and interpret professional journals, technical procedures, and governmental regulations
• Ability to write reports, business correspondence, and procedure manuals
• Strong written and verbal communication skills
• Ability to effectively present information and respond to questions from leaders, team members, and external parties
• Ability to understand, read, write, and speak English
• Strong organizational and time management skills
• Ability to work a flexible schedule including nights and weekends
• Ability to travel
• Ability to maintain confidentiality
• Minimum of 4 days of availability per month required for palliative care positions
• Ability to triage consults by patient acuity

Preferred Qualifications

• Board Certification in Hospice and Palliative Care

Physical Demands and Work Environment

• Regular standing, walking, and manual dexterity required
• Ability to lift and move objects up to 50 pounds
• Close and distance vision, color and peripheral vision, depth perception, and ability to adjust focus
• Exposure to bodily fluids, infectious diseases, and conditions typical to healthcare settings
• Routine use of standard medical equipment and tools associated with clinical care
