Nurse Practitioner - Post Liver Coordinator (Medical Center)

Houston Methodist is seeking a Nurse Practitioner to serve as a Post Liver Coordinator at our Medical Center. In this role, you will function as a licensed advanced practice provider responsible for all aspects of patient care, including diagnosis, treatment, and consultation for a defined patient population. You will work in both inpatient and outpatient settings, performing according to the Nurse Practice Act of the State of Texas as part of an interprofessional treatment team.

Responsibilities

• Coordinate appropriate, timely patient care under physician supervision, focusing on identified expected outcomes individualized to the patient
• Conduct health assessments (initial, ongoing, and discharge as appropriate), identify normal and abnormal health characteristics, develop plans of care, initiate appropriate interventions, and evaluate care outcomes
• Perform invasive, diagnostic, or therapeutic procedures as necessary based on licensure and credentialed privileges
• Order tests and prescribe medications as appropriate
• Serve as a patient's healthcare provider and primary clinical resource
• Coordinate patient care between healthcare providers and participate in interprofessional rounds
• Demonstrate leadership as a clinical resource, interprofessional consultant, and patient advocate
• Identify staff development opportunities and provide coaching to colleagues
• Serve as a preceptor for Nurse Practitioner students and medical residents as applicable
• Advocate on behalf of patients and families to identify and resolve clinical and ethical concerns
• Lead and initiate outcome, quality, and safety improvement activities in alignment with organizational goals
• Implement department strategies to achieve financial targets through timely documentation, decreasing length of stay, and optimizing efficiency
• Promote quality healthcare through integration of current research into practice and participation in evidence-based activities
• Provide education on preventative care and prescribed treatments

Required Qualifications

• Master's degree or higher from an accredited school of Nursing, with graduation from an accredited Nurse Practitioner program
• Two years of registered nurse, advanced practice, or nurse practitioner clinical experience
• Active Texas RN license (Texas Board of Nursing); if establishing Texas residency, permanent Texas license must be obtained within 60 days
• Non-Acute Care state license
• Non-Acute Care ANCC recognized certification
• BLS (Basic Life Support) or Instructor certification from the American Heart Association
• ACLS (Advanced Cardiac Life Support) or Instructor certification from the American Heart Association
• Prescriptive Authority from the Texas Board of Nursing
• DEA Narcotics License from the Drug Enforcement Administration (must be obtained within 6 months)

• Work Attire: Scrubs, business professional, or other department-approved attire
• On-Call: Yes; employees may be required to be on-call during emergencies including disasters and severe weather events
• Travel: May require travel within the Houston Metropolitan area
