The opportunity

Job Description

Integra Vascular is seeking a Nurse Practitioner or Physician Assistant to provide on-site vascular care to residents at client facilities nationwide. In this role, you will perform comprehensive clinical assessments, develop individualized treatment plans, and deliver preventative and therapeutic vascular care to minimize unnecessary wounds, hospitalizations, and patient travel.

Clinical Setting

You will work across skilled nursing facilities and assisted living facilities, providing direct patient care to complex acute, critical, and chronically ill patients. Your work will focus on early diagnosis and exceptional onsite care delivery.

Responsibilities

  • Perform on-site screening clinical assessments to determine appropriate patient treatment plans
  • Conduct age-appropriate history and physical examinations for complex acute, critical, and chronically ill perioperative patients
  • Order and interpret diagnostic and therapeutic tests relative to patient age-specific needs
  • Provide care in accordance with facility, state, and federal regulations
  • Monitor the effectiveness of clinical interventions
  • Maintain professional relationships with facility contacts and stakeholders
  • Educate family members, clients, and prospective accounts regarding services and programs
  • Assist with physician relations development and maintenance as assigned
  • Complete weekly, monthly, and other reports in a timely and comprehensive manner
  • Perform other duties as assigned by the Medical Director

Required Qualifications

  • Current Nurse Practitioner or Physician Assistant license
  • Degree from an accredited Nurse Practitioner or Physician Assistant program
  • ACLS certification
  • Current infection control certification
  • Strong clinical collaboration skills with physicians
  • Proficient computer skills
  • Clean driving record
  • Personally owned vehicle required
  • Not listed on the OIG exclusion list

Preferred Qualifications

  • PACS and EMR experience
  • Vascular care experience, including venous insufficiency and peripheral arterial disease
  • Image-guided injectables experience
  • Advanced geriatric care experience
  • Experience in skilled nursing facilities or assisted living facilities

Travel Requirements

Local travel is required for this position. You must have a personally owned vehicle and a clean driving record.
