The opportunity

Job Description

Innovative Medical Group, a primary care clinic, is seeking a Physician Assistant or Nurse Practitioner to join the Physical Medicine and Rehabilitation (PM&R) team on a part-time, flexible schedule. This role offers meaningful clinical work with schedule flexibility, caring for patients recovering from injury, surgery, stroke, and deconditioning in both clinic and post-acute settings.

Clinical Setting

You will work within a comprehensive primary care clinic that provides internal medicine, adult primary care, women's and men's health, same-day appointments, telehealth visits, and a growing PM&R program. The position includes clinic-based work and rounding on patients in skilled nursing facilities in and around the Yucaipa area up to Yucca Valley.

Responsibilities

  • Evaluate and manage PM&R patients, including new consults and follow-up visits
  • Develop and adjust individualized treatment plans in collaboration with the supervising physician
  • Order and interpret diagnostic studies as appropriate
  • Coordinate care with physical, occupational, and speech therapy teams
  • Perform or assist with in-office procedures as credentialed
  • Document visits accurately and on time in the EHR
  • Round on patients in skilled nursing facilities as scheduled
  • Communicate with patients and families on plan of care and progress

Required Qualifications

  • Current California PA license or California NP license
  • Able to work collaboratively within a physician-led team
  • Strong clinical judgment, communication skills, and documentation habits
  • Reliable transportation for facilities in and around Yucaipa area up to Yucca Valley

Preferred Qualifications

  • Experience in PM&R, orthopedics, neurology, or post-acute care
  • Strong new graduates are considered
