Ennoble Care, a mobile primary care, palliative care, and hospice service provider, is seeking an experienced Advanced Provider (Nurse Practitioner or Physician Assistant) to provide primary care to homebound and palliative care patients in the Augusta, Georgia area on a full-time basis. You will work under physician supervision, caring for a diverse patient population with chronic diseases and limited mobility.

Responsibilities

  • Provide high-quality primary and palliative care to patients in their homes in and around Augusta, GA
  • Assess patient health by performing physical examinations, obtaining and updating medical histories, and developing care plans
  • Develop clinical impressions, diagnoses, and treatment plans
  • Order diagnostic testing as medically necessary
  • Prescribe medications, supplies, and appropriate medically necessary interventions
  • Foster trust and consult with patients, families, and caregivers during visits
  • Communicate effectively with office staff and clinical operations across internal platforms
  • Provide patient-centered care to an assigned patient panel

Required Qualifications

  • Board Certified Nurse Practitioner or Physician Assistant with valid certification in the State of Georgia
  • Strong clinical skills with the ability to accurately and confidently treat, assess, and diagnose
  • Ability to confidently problem solve and operate independently
  • 1-2 years of primary care experience with geriatric patients

Preferred Qualifications

  • Hospice care experience

Schedule and Employment

  • Full-time position with flexible scheduling that works for you

Compensation and Benefits

  • Salary: $105,000-$145,000+ depending on experience and other incentives
