This full-time position offers an opportunity to provide primary care to patients through virtual health modalities on a 2nd shift schedule. You will work as either a Nurse Practitioner or Physician Assistant, assessing and diagnosing patients, determining and adjusting treatment plans, and managing a range of conditions based on clinical indication and evidence-based care.
Responsibilities
- Assess, diagnose, and determine or alter treatment and management plans appropriate for patient age, acuity, and clinical condition via virtual health modalities
- Order, perform, and interpret appropriate diagnostic studies
- Prescribe pharmacologic and non-pharmacologic interventions and therapies
- Manage conditions based on clinical indication, evidence-based care, cost effectiveness, and assessment of risks, benefits, and alternatives
- Leverage digital diagnostics and remote physiologic monitoring when indicated
- Provide health promotion, disease prevention, and disease management counseling and education to patients and families
- Manage patients as part of an interdisciplinary team and within scope of practice
- Seek physician or other healthcare team member consultation or referrals as appropriate
- Escalate need for more emergent or specialized care when necessary
- Maintain accurate, complete, concise, and timely documentation in the electronic medical record
- Substantiate and submit professional services consistent with compliant coding and billing practices
- Facilitate consistent, coordinated care and clear communication among all members of the healthcare team and health or community agencies
- Participate in quality, safety, and peer review initiatives, performance activities, organizational and departmental meetings, committees, and workgroups as necessary
- Participate in education and onboarding of new team members, students, and other healthcare professionals
- Seek experiences to maintain and develop clinical and professional skills and advance the profession
- Maintain standards of productivity, access, face-to-face time, and quality metrics to ensure optimal, safe, and timely patient care delivery
- Demonstrate knowledge and skills necessary to provide age-appropriate care and possess knowledge of growth and development principles across the life span
Required Qualifications
For Advanced Practice Registered Nurse (APRN):
- Active RN license in state(s) of practice
- Active APRN/NP/APRN-FPA/APNP license(s) in state(s) of practice
- Active national board certification as Nurse Practitioner in area of clinical practice and populations served (Family, Adult, or Adult-Gero)
- Active DEA registration as applicable for state of practice
- If practicing in Illinois: active Illinois Controlled Substance License
For Physician Assistant (PA):
- Active PA license(s) in state(s) of practice
- Active PA national board certification from National Commission on Certification of Physician Assistants
- Active DEA registration as applicable for state of practice
- If practicing in Illinois: active Illinois Controlled Substance License
Education
For Advanced Practice Registered Nurse (APRN):
- Master's or Doctorate degree from accredited Nursing Program
For Physician Assistant (PA):
- Bachelor's, Master's, or Doctorate degree from accredited PA Program
Experience
- Typically requires 3 years of experience
