Nurse Practitioner or Physician Assistant - SCC - Providence, Rhode Island

Optum Home & Community Care is seeking a Nurse Practitioner or Physician Assistant to join the Senior Community Care (SCC) program in Providence, Rhode Island. This integrated care delivery program coordinates clinical care for high-risk health plan members in nursing homes, senior housing, and assisted living settings. You will provide comprehensive assessments, support members during sub-acute rehabilitation episodes, and conduct advanced illness and advance care planning conversations.

Clinical Setting

You will work with patients in nursing homes, senior housing, and assisted living settings, providing care that addresses physical, mental, and social needs.

Responsibilities

• Conduct comprehensive assessments including past medical history review, medication reconciliation, vital signs, and physical examination
• Perform evidence-based screenings and point-of-care testing as appropriate
• Support members during short-term sub-acute rehabilitation episodes
• Ensure accurate and complete documentation, including ICD-10 conditions
• Communicate and collaborate with the interdisciplinary care team
• Conduct advanced illness and advanced care planning conversations
• Provide patients and caregivers with counseling and education

Required Qualifications

• For Nurse Practitioners: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• For Physician Assistants: Current NCCPA certification and state licensure
• Active, unrestricted licensure in the state where you will practice (or ability to obtain by start date) without current disciplinary actions or practice limitations
• Active and unrestricted driver's license with access to reliable transportation
• Current, active DEA licensure or prescriptive authority, or ability to obtain post-hire per state regulations (unless prohibited in your state of practice)
• Ability to gain a collaborative practice agreement if applicable in your state
• Ability to navigate varied environments and position yourself as needed to perform job duties

Preferred Qualifications

• One or more years of clinical experience as an Advanced Practice Clinician, preferably in a long-term care setting
• Experience in geriatric medicine, long-term care, senior living, or home care settings
• Experience meeting the medical needs of patients with complex behavioral, social, and/or functional needs
• Understanding of geriatrics and chronic illness
• Understanding of advanced illness and end-of-life discussions
• Proficiency with electronic medical records and technology

Compensation and Benefits

• Compensation generally ranges from $109,500 to $164,000, including base pay and bonus
• Sign-on bonus of $20,000 or student loan repayment of up to $40,000
