Nurse Practitioner or Physician Assistant (PRN) - In-Home Health Assessments*

Matrix Medical Network is seeking a Nurse Practitioner or Physician Assistant to conduct comprehensive in-home health assessments for adult and geriatric patients across the nation. This PRN position is based in Calais, ME and requires regular travel to this area. You will meet patients in their homes or skilled nursing facilities to conduct thorough health evaluations, identify care gaps, and improve health outcomes.

Clinical Setting

You will provide services at members' places of residence, including private homes and skilled nursing facilities. Visits typically last up to one hour and are conducted Sunday through Saturday, 8 am to 7 pm, based on your availability.

Responsibilities

• Conduct adult and geriatric health assessments including medical history, diagnosis, treatment review, and health education
• Review and observe members' current health status, medical history, medication adherence, and social environment
• Administer and order point-of-care and lab-based testing as needed
• Provide physician referrals and case management referrals according to Matrix guidelines and protocols
• Collaborate with primary care physicians, case managers, social workers, family members, and other healthcare personnel
• Provide clear documentation of all assessments and follow-up recommendations
• Educate patients and coordinate follow-up care with primary care physicians

Required Qualifications

• Master's degree or commensurate experience with satisfactory completion of NP licensure
• Current NP licensure in state of practice with prescription authority or ability to obtain prescriptive authority
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
• Current BLS, ACLS, or CPR certification
• Valid state driver's license for state of residence
• Automobile insurance coverage meeting the following minimums for state of residence: Bodily Injury $100,000 per person/$300,000 per incident; Property Damage $50,000; Collision and comprehensive deductibles $1,000 or less
• Strong computer skills and familiarity with electronic health records and medical record software
• Excellent verbal and written communication skills

Preferred Qualifications

• One year of NP experience (new graduates are encouraged to apply)

Travel Requirements

This position requires regular travel to the Calais, ME area. A valid driver's license and personal automobile with appropriate insurance coverage are required.

Schedule and Flexibility

Visits are scheduled based on your availability. You can work during the day, evening, and/or weekends. Assessments are conducted Sunday through Saturday, 8 am to 7 pm.

Compensation and Bonuses

• Sign-on bonus: $1,500
• Per-assessment completion rate: $90 to $115 depending on state of residence
• Monthly earning potential: $1,800 to $11,500 based on completing 20 to 100 visits monthly
