Nurse Practitioner or Physician Assistant - part time (weekend rounding) - $5k sign on bonus

Curana Health is seeking a Nurse Practitioner or Physician Assistant for a part-time weekend rounding position in skilled nursing facilities and senior living communities. You will provide direct primary care to seniors, focusing on wellness promotion, chronic disease management, and care coordination across multiple facility settings.

Clinical Setting

You will work in skilled nursing facilities, assisted living, and independent living communities serving seniors. Curana Health operates across 1,500+ communities in 32 states, supporting 200,000+ seniors through value-based care models.

Responsibilities

• Perform comprehensive assessments, develop treatment plans, and provide ongoing care management for residents with emphasis on preventive care and chronic disease management
• Conduct regular rounds to assess residents' conditions, adjust treatment plans, and address emergent health concerns
• Collaborate with facility staff, family members, and other healthcare providers to ensure comprehensive, coordinated care tailored to residents' needs
• Educate residents, families, and facility staff on disease prevention, wellness, and chronic illness management
• Maintain accurate, timely, and thorough documentation in the electronic medical record system in compliance with regulatory standards and organizational protocols
• Participate in quality improvement initiatives to enhance care delivery, reduce hospital readmissions, and optimize patient satisfaction

Schedule

Part-time weekend rounding position.

Required Qualifications

• Valid Nurse Practitioner or Physician Assistant license in state of operations
• Current board certification in your field
• Master's Degree in Nursing or Physician Assistant Studies
• Excellent written and oral communication skills
• Punctuality, organizational skills, and ability to function within a team

Preferred Qualifications

• Strong leadership, interpersonal, and problem-solving skills

Travel Requirements

This role requires travel to and from one or more assigned facilities. Curana Health may determine that a valid driver's license, current auto insurance, and an acceptable driving record are required. If so, you will receive thirty (30) days' notice of this requirement. Periodic motor vehicle record (MVR) checks may be conducted as a condition of continued employment.
