Optum Home & Community Care is seeking a Nurse Practitioner or Physician Assistant to provide integrated care to high-risk members in the Senior Community Care program. You will work in nursing homes, senior housing, and assisted living settings across Middle River, Bel Air, and Havre De Grace, Maryland, delivering comprehensive clinical care with no on-call, weekend, or holiday requirements.
Clinical Setting
You will serve patients in nursing homes, senior housing, and assisted living environments as part of an integrated care delivery model that addresses physical, mental, and social health needs.
Responsibilities
- Conduct comprehensive assessments including past medical history review, medication reconciliation, vital signs, and physical examination
- Perform evidence-based screenings and point-of-care testing as appropriate
- Support members during short-term sub-acute rehabilitation episodes
- Ensure accurate and complete documentation, including ICD-10 coding
- Communicate and collaborate with the interdisciplinary care team
- Conduct advanced illness and advanced care planning conversations
- Provide patients and caregivers with counseling and education
Required Qualifications
- For Nurse Practitioners: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
- For Physician Assistants: Current NCCPA certification and state licensure
- Active, unrestricted state licensure in Maryland (or ability to obtain by start date) without current disciplinary actions or practice limitations
- Active and unrestricted driver's license with access to reliable transportation
- Current, active DEA licensure and prescriptive authority (or ability to obtain post-hire, per state regulations)
- Ability to gain a collaborative practice agreement if applicable in your state
- Ability to navigate varied environments and position yourself as needed to perform job duties
Preferred Qualifications
- One or more years of clinical experience as an Advanced Practice Clinician, preferably in a long-term care setting
- Experience in geriatric medicine, long-term care, senior living, or home care
- Experience meeting the medical needs of patients with complex behavioral, social, and functional needs
- Understanding of geriatrics and chronic illness management
- Understanding of advanced illness and end-of-life discussions
- Proficiency with electronic medical records and technology
Schedule
No on-call, weekends, or holidays required.
Compensation and Benefits
Compensation generally ranges from $109,500 to $164,000 based on local labor markets, education, work experience, and may increase based on productivity and performance.
Sign-on Bonus: $10,000 for individuals who have not previously participated in this program.
Student Loan Repayment Bonus: $20,000 available for individuals who have not previously participated in this program.
