Nurse Practitioner or Physician Assistant, HouseCalls - Erie Genesee Orleans

Optum HouseCalls is seeking a Nurse Practitioner or Physician Assistant to conduct comprehensive in-home health assessments for Medicare Advantage and other plan members in the Erie, Genesee, and Orleans areas. This is a non-prescribing, field-based role focused on improving health outcomes through education, gap closure, and collaboration with primary care providers.

Responsibilities

• Conduct comprehensive in-home assessments including past medical history review, medication reconciliation, vital signs, and physical examination
• Perform evidence-based screenings and point-of-care testing as appropriate
• Identify and document diagnoses for care management and treatment planning
• Communicate assessment findings to members' primary care providers to address gaps in care
• Recognize urgent and emergent situations and intervene appropriately
• Educate members on disease processes, medications, and medication compliance
• Address social determinants of health and provide referrals as needed

Required Qualifications

• For Nurse Practitioners: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• For Physician Assistants: Current NCCPA certification and state licensure
• Active, unrestricted licensure in the state where you will practice (or ability to obtain by start date) without current disciplinary actions or practice limitations
• Active and unrestricted driver's license with access to reliable transportation to complete home visits
• Ability to navigate varied home environments and perform physical assessments (ability to sit, stand, and kneel as needed)
• If practicing in a state that allows independent practice without a Collaborative Agreement, you must obtain approval from your licensing board if required. New hires who are eligible but have not applied must do so within one month of starting. If not eligible at hire, you must begin working toward eligibility within one month and apply for approval within three months of becoming eligible.

Preferred Qualifications

• One or more years of clinical experience in family medicine, geriatrics, or home health
• Proficiency with electronic medical records and technology
• Ability to transport equipment weighing up to 30 pounds and navigate stairs during home visits
• Proven communication skills with geriatric or Medicare populations
• Fluency in languages other than English

Schedule

Flexible scheduling is available with 20 to 40 hours per week. Providers can flex their time between 8 a.m. and 8 p.m. over the seven-day work week. No on-call, weekend, or holiday requirements.

Compensation and Benefits

Compensation for this specialty generally ranges from $125,000 to $175,000. Total cash compensation includes base pay and bonus and is based on several factors including local labor markets, education, work experience, and may increase over time based on productivity and performance. A comprehensive benefits package is offered including incentive and recognition programs, equity stock purchase, and 401(k) contribution (all benefits subject to eligibility requirements).

Additional compensation may be available for qualified candidates who are fluent in languages other than English and use those language skills during member visits. This is a drug-free workplace and candidates are required to pass a drug test before beginning employment.
