Curana Health is seeking a Nurse Practitioner or Physician Assistant to deliver primary care services to seniors in skilled nursing facilities, assisted living communities, and independent living settings across our national network. This is a full-time position with a 7 on/7 off schedule, offering the opportunity to build meaningful patient relationships while practicing with greater autonomy and clinical independence.
Clinical Setting
You will work directly within senior living communities, providing on-site primary care services to residents. Curana Health operates across 1,500+ communities in 32 states, serving 200,000+ seniors through a value-based care model that emphasizes proactive, relationship-based care and improved health outcomes.
Responsibilities
- Perform comprehensive health assessments and develop individualized treatment plans for residents
- Diagnose, treat, and manage acute and chronic medical conditions with a focus on preventative care and long-term wellness
- Provide ongoing care management that improves quality of life while helping residents remain healthier and more independent
- Conduct regular facility rounds and develop strong relationships with residents, families, and care teams
- Monitor changes in health status and proactively intervene to prevent complications and unnecessary hospitalizations
- Educate residents and caregivers on disease management, preventative care, and wellness strategies
- Partner closely with physicians, clinical staff, facility leadership, specialists, and family members to coordinate comprehensive care plans
- Serve as a trusted clinical resource within the facility and contribute to interdisciplinary care discussions
- Support initiatives focused on improving patient outcomes, enhancing quality measures, and reducing avoidable hospital readmissions
- Maintain accurate and timely documentation within the electronic medical record (EMR)
- Ensure compliance with applicable regulatory, clinical, and organizational standards
Required Qualifications
- Valid Nurse Practitioner or Physician Assistant license in state of operations
- Current board certification in your field
- Master's Degree in Nursing or Physician Assistant Studies
- Excellent written and oral communication skills
- Strong leadership, interpersonal, and problem-solving skills
- Punctuality, organizational skills, and ability to function within a team
Schedule
This position operates on a 7 on/7 off schedule, providing flexibility and work-life balance.
Travel Requirements
This role requires travel to and from one or more assigned facilities. Curana Health may determine in the future that a valid driver's license, current auto insurance, and an acceptable driving record are required. If so, you will be provided with thirty (30) days notice of this requirement. Periodic motor vehicle (MVR) checks may be conducted as a condition of, and/or for, continued employment.
