Curana Health is seeking a Nurse Practitioner or Physician Assistant to provide primary care services in senior living facilities and skilled nursing facilities. In this role, you will deliver direct patient care to seniors, focusing on wellness promotion, chronic disease management, and care coordination across assisted and independent living communities.

Clinical Setting

You will work in senior living communities and skilled nursing facilities serving 200,000+ seniors across 1,500+ communities in 32 states. This is a full-time position that requires travel to and from one or more assigned facilities.

Responsibilities

  • Perform comprehensive assessments, develop treatment plans, and provide ongoing care management for residents with focus on preventive care and chronic disease management
  • Conduct regular rounds to assess residents' conditions, adjust treatment plans, and address emergent health concerns
  • Collaborate with facility staff, family members, and other healthcare providers to ensure comprehensive, coordinated care tailored to residents' needs
  • Educate residents, families, and facility staff on disease prevention, wellness, and managing chronic illnesses
  • Maintain accurate, timely, and thorough documentation in the electronic medical record (EMR) system in compliance with regulatory standards and organizational protocols
  • Participate in quality improvement initiatives to enhance care delivery, reduce hospital readmissions, and optimize patient satisfaction

Required Qualifications

  • Valid Nurse Practitioner or Physician Assistant license in state of operations
  • Current board certification in your field
  • Master's Degree in Nursing or Physician Assistant Studies
  • Excellent written and oral communication skills
  • Strong leadership, interpersonal, and problem-solving skills
  • Punctuality, organizational skills, and ability to function within a team

Travel Requirements

This role requires travel to and from one or more assigned facilities. Depending on the amount of travel required, Curana may determine that a valid driver's license, current auto insurance, and an acceptable driving record are required. If so, you will be provided with thirty (30) days notice of this requirement. Periodic motor vehicle (MVR) checks may be conducted as a condition of continued employment.
