Compassionate Medical Care of WNY is seeking a Nurse Practitioner to join our primary care team in Amherst, NY. In this role, you will provide comprehensive patient care in a supportive, patient-centered environment, working with diverse patient populations to diagnose and treat acute and chronic medical conditions.
Responsibilities
- Conduct comprehensive patient assessments, physical exams, and health histories
- Diagnose and treat acute and chronic medical conditions across patient populations
- Develop and manage individualized patient care plans
- Prescribe medications and therapies in accordance with clinical guidelines
- Order and interpret diagnostic tests, labs, and imaging
- Educate patients and families on health management, prevention, and wellness
- Collaborate with physicians, specialists, and care teams to ensure coordinated care
- Maintain accurate and timely documentation in the EMR system
Required Qualifications
- Active Nurse Practitioner license in the State of New York
- Master's or Doctoral degree in Nursing (MSN or DNP) from an accredited program
- Current national NP certification (FNP, ANP, or relevant specialty)
- DEA registration and prescriptive authority in New York
- Proficiency with electronic medical records (EMR) systems
- Strong interpersonal and communication skills with a patient-first approach
- Ability to work both independently and collaboratively within a multidisciplinary team
