The opportunity

Job Description

Cognizant Behavioral Health Services, an established outpatient behavioral health practice in Chadds Ford, Pennsylvania, is seeking an Advanced Practice Provider—Psychiatric Mental Health Nurse Practitioner (PMHNP) or Physician Assistant-Certified (PA-C)—to provide psychiatric evaluations, medication management, and ongoing behavioral health treatment. You will work in a collaborative, team-based environment alongside psychiatrists, therapists, and support staff, treating a diverse patient population with a broad range of behavioral health needs throughout Delaware County and surrounding communities.

Responsibilities

  • Conduct comprehensive psychiatric evaluations and assessments
  • Diagnose and treat a variety of behavioral health conditions
  • Develop individualized treatment plans based on each patient's clinical needs, history, and goals
  • Prescribe and manage psychiatric medications within your scope of practice
  • Monitor patient progress, medication effectiveness, and potential side effects
  • Provide patient and family education regarding diagnoses, medications, and treatment recommendations
  • Collaborate with psychiatrists, therapists, and other members of the treatment team
  • Maintain accurate and timely documentation in the electronic health record
  • Coordinate care with primary care providers, specialists, and community resources when appropriate
  • Follow all clinical, ethical, regulatory, and organizational standards

Required Qualifications

  • Graduate of an accredited nurse practitioner or physician assistant program
  • Current certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP) or Physician Assistant-Certified (PA-C)
  • Current or eligible Pennsylvania professional license
  • Current or eligible DEA registration, as required for the position
  • Three years of experience
  • Strong clinical assessment, communication, and documentation skills
  • Ability to work independently while contributing to a collaborative clinical team
