The opportunity

Job Description

Oak Street Health is seeking a Nurse Practitioner to provide primary care to adults on Medicare in an outpatient clinical setting. You will see patients independently and collaborate with physicians based on individual patient needs, building meaningful long-term relationships while delivering evidence-based, value-based care focused on keeping patients healthy and out of the hospital.

You will practice as part of a collaborative care team that includes a physician, nurse practitioner or physician assistant, medical assistants, a dedicated medical scribe, and support from registered nurses and care managers. The practice partners with a network of specialists and hospitals for specialty and acute care, allowing you to focus on primary care, care coordination, and population health within the clinic.

Clinical Setting

This is a full-time outpatient primary care position located at 6401 E Washington St, Indianapolis, IN 46219. You will work Monday through Friday, 8:00 AM to 5:00 PM, with 40 anticipated weekly hours.

Responsibilities

  • Provide exceptional primary care through office visits for routine and acute issues
  • Conduct annual wellness visits and health risk assessments with holistic health evaluation and thorough documentation
  • Coordinate care with other providers, specialists, testing facilities, and agencies
  • Lead population health initiatives in coordination with the care team, such as ensuring eligible patients receive evidence-based preventive screenings
  • Assist the care team with phone triage and patient outreach
  • Educate patients on their health conditions, care plans, and treatments
  • Conduct home visits as needed
  • Participate in Oak Street Health promotional activities
  • Perform other duties as assigned

This role reports to the Center Medical Director and works closely with operational leadership.

Required Qualifications

  • Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
  • National certification in one of the following specialties:
  • Family Nurse Practitioner
  • Adult-Gerontology Primary Care Nurse Practitioner
  • Adult Nurse Practitioner
  • Gerontological Nurse Practitioner
  • Active, non-probationary state Nurse Practitioner license
  • Active DEA license
  • US Work Authorization

Preferred Qualifications

  • Experience in primary care: internal medicine, geriatrics, or family medicine
  • Understanding of HCC (Hierarchical Condition Categories) documentation, ICD-10 coding, and Health Risk Assessments (HRAs)
  • Passion for teamwork and cross-functional collaboration
  • Interest in innovative models focused on empirically-guided population health
  • Bilingual proficiency in applicable areas

Compensation and Benefits

Salary range: $87,035 to $187,460 annually, with typical range of $95,000 to $148,500. Actual base salary will depend on experience, education, geography, and other relevant factors.
