Cedars-Sinai's Comprehensive Transplant Center is seeking a Nurse Practitioner to join the Post-Liver Transplant team. This full-time position offers the opportunity to provide specialized care to patients following liver transplantation while collaborating with physicians, nurses, transplant specialists, and other healthcare professionals throughout the continuum of care.
Clinical Setting
The Comprehensive Transplant Center provides advanced, multidisciplinary care for patients with complex organ disease and those undergoing transplantation. The center brings together specialists across lung, heart, liver, kidney, and pancreas transplantation to support patients throughout their care journey. The department is committed to delivering high-quality, compassionate, and coordinated care from initial evaluation through transplantation and long-term follow-up.
Responsibilities
- Collaborate with a multidisciplinary healthcare team to provide comprehensive care to patients status post liver transplant
- Conduct patient assessments, including medical histories, physical examinations, and review of diagnostic testing
- Develop individualized care plans in collaboration with the clinical team and communicate plans clearly to patients and families
- Perform procedures within the Nurse Practitioner's authorized scope of practice, training, competency, and applicable Cedars-Sinai policies
- Monitor patient progress and collaborate with the care team to modify treatment plans as clinically appropriate
- Educate patients and families regarding disease management, medications, lifestyle modifications, and ongoing care needs
- Support continuity of care throughout evaluation, treatment, and follow-up
- Participate in quality improvement, evidence-based practice, and clinical research initiatives as applicable to the role
- Maintain accurate and timely clinical documentation while supporting established quality and patient-safety standards
Schedule
- Full-time position with 10-hour days
