The opportunity

Job Description

A medical group providing primary care services to nursing homes is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to provide ongoing primary care coverage at a skilled nursing facility in Canarsie, Brooklyn. This full-time position offers the opportunity to manage a consistent patient panel of geriatric nursing home residents while working closely with nursing staff and facility leadership to ensure continuity of care for both routine and acute medical needs.

Clinical Setting

You will work in a skilled nursing facility (SNF) environment, providing primary care services to geriatric residents with multiple chronic medical conditions.

Responsibilities

  • Provide primary care services to geriatric nursing home residents
  • Conduct routine, follow-up, and urgent patient visits
  • Evaluate and manage acute medical concerns
  • Manage patients with multiple chronic medical conditions
  • Review medications, labs, and treatment plans
  • Coordinate care with nursing staff, specialists, and facility leadership
  • Participate in Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs when applicable
  • Maintain accurate and timely clinical documentation using SigmaCare or PCC

Required Qualifications

  • Active New York Nurse Practitioner or Physician Assistant license
  • Active Medicare and Medicaid provider ID
  • Strong clinical judgment and ability to work independently
  • Comfortable managing a consistent nursing home patient panel

Preferred Qualifications

  • Experience working with geriatric patients
  • Skilled Nursing Facility (SNF) or long-term care experience
  • Previous RN experience in a nursing home or long-term care setting

Schedule

  • Full-time, Monday through Friday
  • Availability between 8:00 AM and 6:00 PM
  • Consistent facility coverage and patient panel

Compensation

  • $140,000 to $160,000 annual salary
