Medical Director Services PC is seeking an experienced Nurse Practitioner or Physician Assistant for a skilled nursing facility in Gardner, MA. New graduates are welcome with extensive training provided. You will deliver quality patient care in a skilled nursing setting, working collaboratively with physicians and the healthcare team to manage resident care, reduce unnecessary hospitalizations, and optimize medication management.

Responsibilities

  • Assess patient health by interviewing patients, performing physical examinations, and obtaining and updating medical histories
  • Develop and implement patient management plans to provide continuity of care
  • Perform therapeutic procedures including administering injections and immunizations, suturing, and managing wounds and infections
  • Document patient care services by charting in patient and department records
  • Instruct and counsel patients on therapeutic regimens, health maintenance, and wellness promotion
  • Contribute to physician effectiveness by identifying patient care issues, providing clinical information, and recommending courses of action
  • Maintain safe and clean working environments by complying with procedures, rules, and regulations
  • Protect patients and employees by adhering to infection-control policies and protocols
  • Comply with federal, state, and local legal and professional requirements
  • Maintain professional and technical knowledge by attending educational workshops and reviewing professional publications
  • Develop healthcare team staff by providing information and educational opportunities
  • Contribute to team effort by accomplishing related results as needed
  • Perform other duties as assigned
