Nurse Practitioner NP or Physician Assistant PA Skilled Nursing Facility Rounds

We are seeking a licensed Nurse Practitioner (NP) or Physician Assistant (PA) to join our team providing clinical care in skilled nursing facilities across Maryland. You will travel to assigned facilities to conduct monthly patient visits, review charts, update care plans, and collaborate with facility staff to ensure continuity of care.

Responsibilities

• Conduct monthly visits on assigned patients in skilled nursing facilities
• Review patient charts, document findings, and update care plans as needed
• Collaborate with facility staff to ensure continuity of care
• Maintain compliance with all state, federal, and company documentation standards

Required Qualifications

• Active Maryland NP or PA license
• Reliable transportation
• Strong communication and clinical documentation skills

Preferred Qualifications

• Experience in geriatrics, long-term care, or internal medicine

Travel Requirements

• Willingness to travel to multiple facilities across Maryland
