Nurse Practitioner (NP) – Home Health Care (Northern New Jersey)

This position offers an opportunity to provide primary care to homebound patients throughout Northern New Jersey as a Nurse Practitioner. You will deliver high-quality, patient-centered care directly in patients' homes, building relationships and managing their health conditions with independence and clinical autonomy.

Responsibilities

• Serve as the primary provider for 5–6 homebound patients per day
• Perform patient assessments, vital signs monitoring, and physical examinations
• Diagnose health conditions and illnesses
• Prescribe and manage medications
• Maintain accurate clinical notes and visit schedules
• Build trust and provide guidance to patients and families
• Educate patients on conditions, prevention, and overall health habits
• Deliver patient-centered, compassionate care

Required Qualifications

• Active New Jersey NP license
• DEA license
• Malpractice coverage
• Strong communication skills and professionalism
• Ability to work independently
• Proven NP experience in primary care, ER, clinic, or nursing home settings

Compensation and Benefits

• Salary: $130,000–$140,000 per year
