Nurse Practitioner (NP) - Advanced Practice Provider

Hamilton Health Box is seeking a Nurse Practitioner to provide primary care services across hybrid telemedicine-enabled clinics serving small and medium businesses, as well as rural health clinics. In this role, you will work collaboratively with a supervising physician to deliver both virtual and in-person medical care, focusing on protocol-driven care, health coaching, disease management, preventative screening, and patient education. You will also assist with urgent and emergent matters while helping to develop clinical protocols and coordinate population health management initiatives.

Responsibilities

• Promote and maintain patient health by providing medical services in collaboration with a physician
• Assess patient health by interviewing patients, performing physical examinations, and obtaining and reviewing medical histories
• Determine abnormal conditions by administering or ordering diagnostic tests such as X-rays and laboratory studies, and interpreting results
• Perform therapeutic procedures including administering injections and immunizations, suturing, and managing wounds and infections
• Implement physician directives and recommend options and courses of action for patient care
• Document patient care services by charting patient and department records
• Instruct and counsel patients on normal growth and development, and provide counseling on emotional and daily living concerns
• Develop and implement patient-management plans
• Collaborate in the development and evaluation of patient care protocols and clinical guidelines
• Work autonomously and accept accountability for outcomes; sign orders in accordance with institution and state board requirements
• Utilize telemedicine interface to complete virtual visits
• Float and round to multiple facilities as needed
• Take calls on a rotating schedule, generally one week on every 3-4 weeks with no after-hours onsite requirements while on call
• Maintain compliance with infection-control policies and federal, state, and local legal and professional requirements
• Maintain professional and technical knowledge by attending educational workshops, reviewing professional publications, and participating in professional societies
• Contribute to healthcare team development by providing information and educational opportunities

Required Qualifications

• Current licensure as a Nurse Practitioner (NP)
• Maintenance of licensure and certification required
• Active Basic Life Support (BLS) certification
• Minimum 2 years of primary care, family medicine, internal medicine, urgent care, or comparable outpatient experience
• Current DEA registration or ability to obtain it, if required for the position

Preferred Qualifications

• Bilingual Spanish/English proficiency
• Willingness to obtain licensure to practice in other states

Schedule

• On-site, weekdays, day shift
• Rotating on-call schedule (approximately one week on every 3-4 weeks)
