Nurse Practitioner (MT License required)

Greater Good Health is seeking a Nurse Practitioner to lead patient care at our primary care clinic in Great Falls, Montana. You will manage a panel of senior patients in a value-based care model that prioritizes quality outcomes and high-touch, coordinated care over volume. This role offers the opportunity to deliver meaningful, holistic primary care to older adults in an underserved community while working collaboratively with a supportive care team.

Clinical Setting

You will work at Greater Good Health's Great Falls Clinic, a senior-focused primary care practice. The clinic operates with an integrated care team including Medical Assistants, LPNs, Front Office Coordinators, a Clinic Manager, and a Medical Director. The organization is committed to value-based care delivery and outcomes-focused practice.

Responsibilities

• Manage a patient panel and deliver full scope of primary care services including well visits, acute care, and chronic condition management
• Take medical histories, examine patients, make diagnoses, and prescribe medications within state guidelines
• Perform Annual Wellness Visits and Health Risk Assessments with accurate clinical coding and documentation
• Conduct visits through multiple modalities including in-clinic, telehealth, and potentially home visits
• Order diagnostic tests and interpret results
• Perform appropriate screenings, tests, and in-office procedures
• Provide patient education on preventive measures, lifestyle management, and disease management
• Make referrals to specialists, testing facilities, and other providers as needed
• Develop and manage relationships with specialty consultants and monitor follow-up care
• Monitor post-hospitalization status and conduct post-discharge follow-up visits
• Assist with telephonic triage and outreach
• Provide after-hours coverage on a rotation basis with other clinicians
• Perform prior authorizations and peer-to-peer reviews
• Ensure accurate HCC coding to comprehensively capture patient health conditions
• Meet key quality and clinical outcome measures
• Maintain accurate, complete, and timely charting in the Electronic Health Record
• Lead and participate in care team meetings to discuss patient care and clinic operations
• Help onboard new Nurse Practitioners and precept NP students as applicable
• Participate in regular chart audits and trainings
• Collaborate on ad-hoc projects and business development initiatives
• Support and participate in Greater Good Health-sponsored educational events
• Attend and participate in regular staff operational and educational meetings
• Promote high-quality patient care and outstanding patient experience
• Communicate expertly with all team members to ensure smooth patient flow and experience

Required Qualifications

• Active FNP or AGNP license in the state of Montana
• CPR/BLS Certification
• Prescriptive Authority by the Montana state board of Nursing or Medical Examiners
• DEA License
• Three to five or more years of practice in Primary Care with independent patient care experience
• Valid Montana Driver's License
• Proficiency with Microsoft Word, Excel, and PowerPoint

• Experience delivering educational sessions
