Nurse Practitioner

Minnesota Hospice is seeking a Nurse Practitioner to provide advanced clinical care and medical oversight to hospice patients and families across Dakota, Scott, and Rice counties. In this role, you will utilize advanced clinical expertise and strong communication skills to support patients throughout their hospice journey, serving as a key liaison for evolving medical information and ensuring timely assessment and delivery of appropriate care.

Clinical Setting

Minnesota Hospice is a locally owned and operated community hospice committed to creating a warm, welcoming, and supportive environment. You will work collaboratively with an interdisciplinary team including nursing, social work, chaplaincy, and other specialists to promote quality outcomes and maintain patient comfort in alignment with hospice philosophy.

Responsibilities

• Assess the physical, functional, and psychosocial status of hospice patients through comprehensive evaluations
• Make clinical recommendations to the Medical Director regarding patient eligibility for hospice certification and recertification
• Conduct required face-to-face visits to ensure continued medical eligibility in accordance with regulatory guidelines
• Provide direct medical care to patients within the scope of Nurse Practitioner practice, focusing on symptom management, comfort, and quality of life
• Document all patient encounters, assessments, and clinical decisions accurately in the electronic health record (EHR)
• Communicate changes in patient condition, prognosis, and care needs promptly and effectively with interdisciplinary team members
• Collaborate with nursing, social work, chaplaincy, and other team members to develop and adjust individualized plans of care
• Participate in as-needed patient visits to support ongoing clinical oversight and patient needs
• Participate in a provider on-call rotation to ensure timely clinical support and responsiveness to patient needs
• Review and interpret clinical data, including medications, lab results, and disease progression, to guide care planning
• Support advance care planning discussions and ensure care aligns with patient and family goals
• Provide education to patients, families, and caregivers regarding disease progression, symptom management, and end-of-life expectations
• Ensure compliance with hospice regulations, organizational policies, and best practice standards
• Assist with coordination of care transitions, including admissions, recertifications, and discharges as appropriate
• Maintain strong, professional relationships with patients, families, referral sources, and interdisciplinary team members to promote continuity of care and collaboration

Required Qualifications

• Master's or Doctoral degree in Nursing from an accredited nursing program
• Current, active licensure as a Registered Nurse (RN) and Nurse Practitioner (NP) in Minnesota
• Board certification as a Nurse Practitioner in an appropriate specialty area
• Minimum of three years of clinical experience as a practicing Nurse Practitioner
• Current DEA registration and ability to prescribe medications in accordance with state and federal regulations
• Current CPR certification
• Valid driver's license and auto insurance
• Knowledge of HIPAA and confidentiality requirements
• Knowledge of Medicare Conditions of Participation (CoPs)
• Ability to maintain accurate documentation
• Ability to work both as part of a team and independently
• Strong organizational and time management skills
• Knowledge of Microsoft products (Excel, Word, etc.)

Preferred Qualifications

• Hospice or palliative care experience

Physical Requirements

• Able to regularly lift and move up to 50 pounds
• Regularly operate and assist with physical and mechanical transfers
• May have prolonged periods of sitting at a desk and working on a computer
• Personal protective equipment will be provided, including masks
