Kinetic Medical Group is seeking a full-time Nurse Practitioner in Physical Medicine & Rehabilitation to join a skilled nursing and rehabilitation facility in Huntersville, NC. In this role, you will provide comprehensive medical care to geriatric and rehabilitation patients, managing their rehabilitation and physical medicine needs with clinical autonomy and support from an NP-owned practice.
Clinical Setting
You will work in a skilled nursing facility (SNF) serving geriatric and rehabilitation patients. The practice is owned and operated by nurse practitioners who understand the unique needs and challenges of NP practice.
Responsibilities
- Provide comprehensive medical care to geriatric and rehabilitation patients within the SNF setting
- Evaluate and manage patients' rehabilitation and physical medicine needs
- Develop and monitor individualized treatment plans
- Identify and address medical issues before they become emergencies
- Collaborate closely with nursing, physical therapy, occupational therapy, speech therapy, administration, and families
- Participate in preventative care and quality-improvement initiatives
- Help reduce avoidable hospitalizations and improve patient outcomes
- Communicate effectively with patients, families, and the interdisciplinary care team
- Serve as a clinical resource and trusted partner to the facility
- Practice with a high degree of autonomy while having support from the KMG team
