This Advanced Practice Nurse position is based in East Longmeadow, MA, providing patient care within a bridge care setting. You will conduct patient visits in accordance with local, state, and federal regulations, communicate findings to the interdisciplinary team, and contribute to comprehensive patient care planning.
Responsibilities
- Conduct visits with patients in accordance with local, state, and federal regulations
- Communicate visit findings to designated members of the interdisciplinary team
- Facilitate access to appropriate support models of care, including rehabilitation and ongoing aggressive treatment
- Consult with interdisciplinary team members to contribute to the patient's plan of care
- Prepare and maintain accurate patient records, charts, and documents
- Assist in development of clinical practice guidelines and standards in support of quality care as needed
- Ensure collaboration with patients, caregivers, medical directors, and interdisciplinary team members
Required Qualifications
- Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program
- Active Nurse Practitioner license in the state of operations
- Minimum 1+ years of experience working within Family Practice, General Practice, Geriatric care, or related field
- Valid driver's license and reliable source of transportation
- Willingness to travel
Preferred Qualifications
- Experience in a nursing home, hospital, assisted living, or acute rehab setting
Compensation
- Salary range: $125,000 to $135,000 per year
