The opportunity

Job Description

This Advanced Practice Nurse position is based in East Longmeadow, MA, providing patient care within a bridge care setting. You will conduct patient visits in accordance with local, state, and federal regulations, communicate findings to the interdisciplinary team, and contribute to comprehensive patient care planning.

Responsibilities

  • Conduct visits with patients in accordance with local, state, and federal regulations
  • Communicate visit findings to designated members of the interdisciplinary team
  • Facilitate access to appropriate support models of care, including rehabilitation and ongoing aggressive treatment
  • Consult with interdisciplinary team members to contribute to the patient's plan of care
  • Prepare and maintain accurate patient records, charts, and documents
  • Assist in development of clinical practice guidelines and standards in support of quality care as needed
  • Ensure collaboration with patients, caregivers, medical directors, and interdisciplinary team members

Required Qualifications

  • Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program
  • Active Nurse Practitioner license in the state of operations
  • Minimum 1+ years of experience working within Family Practice, General Practice, Geriatric care, or related field
  • Valid driver's license and reliable source of transportation
  • Willingness to travel

  • Experience in a nursing home, hospital, assisted living, or acute rehab setting

Compensation

  • Salary range: $125,000 to $135,000 per year
