Holisticare Hospice is hiring per diem Nurse Practitioners to deliver comprehensive hospice and palliative care in the Atlantic, Cape, and Cumberland County areas. In this role, you will evaluate and assess patient health histories, diagnose medical conditions, manage terminal illnesses, and develop personalized care plans in collaboration with patients, families, and the interdisciplinary healthcare team.
Responsibilities
- Provide comprehensive medical care and palliative management for patients with terminal illnesses, ensuring compliance with the state Nurse Practice Act under physician supervision
- Consult with the patient's attending physician, hospice medical director, and other healthcare practitioners to assess hospice eligibility and ongoing care needs
- Review clinical information and coordinate patient care with the hospice interdisciplinary team and other healthcare providers
- Engage in patient and family education and counseling regarding preventative care, medical problems, psychological issues, and spiritual concerns
- Collaborate with the interdisciplinary team to identify care goals and address the total needs of patients and their families
- Provide recommendations to the Hospice Medical Director regarding patient eligibility for initial certification and recertification for hospice care
- Maintain current knowledge of Medicare/Medicaid, state, and federal regulations for hospice services
Required Qualifications
- Master's degree
- Current unencumbered license in the State of Practice as a Registered Nurse and Nurse Practitioner
- Minimum one year of hospice or palliative care experience as an RN or NP
- Excellent verbal and written communication abilities
- Computer proficiency including experience with EMR systems
- Current driver's license and vehicle insurance with access to dependable transportation
- CPR certification if required by state
Preferred Qualifications
- Certification in a specialist area such as Hospice and Palliative Nursing (CHPN), Pediatrics, or Geriatrics
Schedule and Compensation
- Per diem employment
- Compensation: $150 per visit
