Hoag Hospital is seeking a Nurse Practitioner to provide supplemental medical care to patients in the home health setting, with a focus on hospice and palliative care. In this role, you will assist in coordinating seamless multidisciplinary patient care from admission through discharge, working to ensure quality outcomes and positive patient experiences.
Responsibilities
- Perform history and physical examinations
- Diagnose and treat medical problems
- Order and interpret diagnostic and laboratory tests
- Perform minor procedures
- Facilitate the discharge process
- Coordinate care across the multidisciplinary team
- Maintain accurate and timely patient documentation
Clinical Setting
This is an in-person position based in home health, serving patients receiving hospice and palliative care services.
Schedule
- Day shift
- Monday to Friday
Required Qualifications
- Master's degree in Nursing
- At least 1-2 years of clinical Nurse Practitioner experience
- Advanced clinical knowledge and skills in the care and management of patients
- Solid computer technical skills, including proficiency with Microsoft Office products (Outlook, Word, Excel, PowerPoint, and Visio)
- Current license in good standing to practice as a Registered Nurse/Nurse Practitioner in California
- Furnishing license issued by the State of California
- Personal DEA number
- BLS and ACLS certification
- National board certification as Nurse Practitioner in area of specialty
Preferred Qualifications
- Two years recent clinical experience as a Nurse Practitioner, preferably within an ambulatory and acute healthcare environment
- Experience related to the care of orthopedic patients
- Qualified bilingual or multilingual candidates
Travel Requirements
This position may require local travel between various organization locations.
