NURSE PRACTITIONER - Hematology Oncology - Mount Sinai Chelsea - Per Diem Days

Mount Sinai Chelsea is seeking a Nurse Practitioner for the Hematology Oncology department to work on a per diem, as-needed basis. In this role, you will provide patient care delivery in collaboration with physicians, managing a select patient population across the hospital continuum.

Clinical Responsibilities

• Develop and implement collaborative practice agreements in hematology oncology and conduct periodic reviews
• Assess patient and family health status through comprehensive health history and physical examination
• Initiate selected therapeutic and diagnostic procedures, consultations, and laboratory examinations
• Prescribe and alter medication regimens according to collaborative practice agreement and protocols
• Interpret clinical findings and adjust the plan of care as appropriate
• Provide teaching and counseling in health promotion, disease prevention, and management of acute and chronic illnesses
• Document findings and plan of care in patient medical records
• Consult with collaborating physician as established in the practice agreement
• Initiate referrals to specialty services and foster continuity of care
• Participate in team meetings and interdisciplinary conferences
• Conduct patient record reviews with the collaborating physician at least every three months
• Manage clinical specialty programs and provide outcome data for program evaluation and performance improvement

Collaborative Practice Requirements

Your practice will be governed by a Collaborative Practice Agreement under New York State Law Chapter 257. Practice protocols will identify the area of practice to be performed in collaboration with a physician and will reflect accepted standards of nursing and medical practice. Protocols include provisions for case management, diagnosis, treatment, and appropriate recordkeeping.

As a Nurse Practitioner, you will:

• Have patients assigned according to patient needs
• Order laboratory tests according to initial work-up guidelines; routine diagnostic procedures do not require collaboration, but consultation and documentation are required before extensive tests such as nuclear medicine, MRI, CT scan, or IVP
• Consult with and refer patients to subspecialty clinics as needed
• Prescribe medications according to New York State Laws and Regulations
• Document findings, consultations, and plans of care in patient medical records

Required Qualifications

• Master's degree from a program designed to prepare a registered nurse to provide acute or primary health care services, or successful completion of an approved supplemental education program (post-Master's NP certificate)
• New York State Nurse Practitioner certification

Compensation

• Hourly rate: $75.00
• Actual salaries depend on experience, education, and operational need
