Nurse Practitioner-Geriatric/Palliative SC - ISM, Mount Sinai Hospital - Full Time - Day

Mount Sinai Hospital is seeking a Nurse Practitioner specializing in Geriatric and Palliative Care for the Icahn School of Medicine. This is a full-time, day-shift position within the clinical practice setting. As an independent practitioner, you will diagnose, treat, and prescribe for patients within your specialty area of practice. Medical orders, progress notes, and treatment plans do not require physician countersignature.

Responsibilities

• Assess patient and family health status through comprehensive health history and physical examination
• Initiate and order therapeutic and diagnostic procedures, consultations, and laboratory examinations
• Prescribe medication regimens and interpret findings according to accepted standards of care and practice guidelines
• Educate and counsel patients in health promotion, disease prevention, and management of acute and chronic illnesses
• Document findings and plan of care in patient medical records
• Consult with physicians according to established standards, policies, procedures, and protocols
• Demonstrate knowledge and skills appropriate to the age of patients served in the assigned area
• Initiate referrals to specialty services and foster continuity of care
• Participate in team meetings and interdisciplinary conferences
• Maintain patient and employee confidentiality
• Develop individualized plans of care based on diagnoses and patient needs
• Record, report, and interpret patient responses to the plan of care
• Perform patient record reviews with the collaborating physician at least every three months if utilizing a collaborative practice agreement
• Complete Focused Professional Practice Proctoring Evaluation (FPPE) or Ongoing Professional Practice Proctoring Evaluation (OPPE) as required by Medical Staff
• Practice as a member of the allied health staff according to Medical Staff rules and Hospital bylaws
• Serve as a resource person to nurses and other health team members
• Participate in peer review and approved nursing and medical research studies
• Keep knowledge current through self-directed learning, literature review, and educational programs
• Participate in departmental and interdisciplinary committees that influence nursing practice and patient care delivery

Required Qualifications

• Active New York State RN license with current registration
• Certification as a Nurse Practitioner by the New York State Department of Education with current registration
• Graduate of a New York State registered Nurse Practitioner program or equivalent, or certification as a Nurse Practitioner by a national association via examination
• Completion of a pharmacy component of not less than required semester hours
• Instruction in New York State and Federal laws relating to prescriptions and recordkeeping (3-hour course)
• Collaborative Practice Agreement with a physician and designated protocols if less than 3,600 hours of experience as a practicing Nurse Practitioner, OR Collaborative Relationship with a physician based on employment in the NP role at Mount Sinai Hospital if documented work history as a Nurse Practitioner with greater than 3,600 practice hours
• Master's degree program designed to prepare an NP to diagnose, treat, and prescribe, or successful completion of an approved supplemental education program (post-master's NP certificate)
• Previous progressive experience relevant to the area of clinical practice
• National Board Certification in Advanced Practice Nursing (required for all NPs hired effective June 1, 2019, or for NPs who bill for professional services; may be deferred up to 1 year if approved by Chief Nursing Officer or designee)
• Credentialing through the Credentialing Committee of the Medical Board with approved Delineation of Privileges
• Drug Enforcement Administration (DEA) certificate
• Basic Life Support (BLS) certification
• Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS), Neonatal Advanced Life Support (NALS), and The Neonatal Resuscitation Program (NRP) certificates as required by specialty
• Physical stamina and manual dexterity to perform a variety of tasks and skills in the area of assignment
• Ability to perform history taking and physical assessment techniques
• Ability to interpret and apply principles of nursing research utilization
• Ability to manage and coordinate a patient caseload
• Excellent interpersonal skills, effective communication skills, creative problem solving, critical thinking, and leadership

Compensation

• Hourly rate: $84.59
• Actual salaries depend on experience, education, and operational need
• Compensation does not include bonuses, incentive pay, differential pay, or other forms of compensation or benefits
• Collective bargaining unit: NYSNA-MSH, G17 - Geriatric/Palliative SC - ISM
