Nurse Practitioner-Geriatric/Palliative SC - ISM, Mount Sinai Hospital - Full Time - Day

Mount Sinai Hospital is seeking a Nurse Practitioner specializing in Geriatric and Palliative Care for a full-time day position in the Icahn School of Medicine. As an independent practitioner, you will diagnose, treat, and prescribe for patients within your specialty area of practice. Medical orders, progress notes, and treatment plans do not require physician countersignature.

Clinical Responsibilities

• Assess patient and family health status through comprehensive health history and physical examination
• Order and initiate therapeutic and diagnostic procedures, consultations, and laboratory examinations
• Prescribe medication regimens and interpret findings according to accepted standards of care and practice guidelines
• Educate and counsel patients in health promotion, disease prevention, and management of acute and chronic illnesses
• Develop individualized plans of care based on diagnoses and patient needs
• Document findings and plans of care in patient medical records
• Consult with physicians according to established standards, policies, procedures, and protocols
• Initiate referrals to specialty services and foster continuity of care
• Participate in team meetings and interdisciplinary conferences
• Maintain patient and employee confidentiality
• Record, report, and interpret patient responses to the plan of care
• Perform patient record reviews with the collaborating physician at least every three months if utilizing a collaborative practice agreement
• Complete Focused Professional Practice Proctoring Evaluation (FPPE) or Ongoing Professional Practice Proctoring Evaluation (OPPE) as required by Medical Staff
• Practice as a member of the allied health staff according to Medical Staff rules and Hospital bylaws

Required Qualifications

• Active New York State RN license with current registration
• Certification as a Nurse Practitioner by the New York State Department of Education with current registration
• Master's degree from a New York State registered Nurse Practitioner program or equivalent, or successful completion of an approved supplemental education program (post-master's NP certificate)
• Completion of a pharmacy component with required semester hours and instruction in New York State and Federal laws relating to prescriptions and recordkeeping (3-hour course)
• Collaborative Practice Agreement with a physician and designated protocols if you have fewer than 3,600 hours of experience as a practicing Nurse Practitioner, OR Collaborative Relationship with a physician based on employment in the NP role at Mount Sinai Hospital if you have documented work history as a Nurse Practitioner with greater than 3,600 practice hours (all NPs will be placed on FPPE initially)
• National Board Certification in Advanced Practice Nursing (required for all NPs hired effective June 1, 2019, or for NPs who bill for professional services regardless of hire date; may be deferred up to 1 year if approved by Chief Nursing Officer or designee)
• Credentialing through the Credentialing Committee of the Medical Board with approved Delineation of Privileges
• Drug Enforcement Administration (DEA) certificate
• Basic Life Support (BLS) certification
• Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS), Neonatal Advanced Life Support (NALS), and Neonatal Resuscitation Program (NRP) certificates as required by specialty
• Previous progressive experience relevant to geriatric and palliative care clinical practice

• Ability to perform history taking and physical assessment techniques
• Ability to demonstrate and document competency in specialty-specific procedures according to established standards as part of initial orientation and annual performance review
• Ability to interpret and apply principles of nursing research utilization
• Ability to manage and coordinate a patient caseload
• Physical stamina and manual dexterity to perform a variety of tasks and skills in your area of assignment
• Excellent interpersonal skills, effective communication skills, creative problem solving, critical thinking, and leadership abilities
• Ability to work within a Collaborative Practice Agreement or Collaborative Practice Relationship Attestation as a member of the health care team

Compensation

• Hourly rate: $84.59
• Actual salaries depend on experience, education, and operational need
• Salary does not include bonuses, incentive pay, differential pay, or other forms of compensation or benefits
