Matrix Medical Network is seeking a full-time Nurse Practitioner to conduct comprehensive in-home health assessments for adult and geriatric patients across the nation. During visits lasting up to one hour, you will review and observe members' current health status, medical history, medication adherence, social environment, and other risk factors to identify care gaps and improve health outcomes. This role offers the opportunity to make a direct clinical impact while working primarily Monday through Friday, 8:00 AM to 5:30 PM.
Clinical Setting
You will provide services at members' places of residence, including private homes and skilled nursing facilities. You will work with physicians, case managers, social workers, family members, and other healthcare personnel to coordinate comprehensive care.
Responsibilities
- Conduct adult and geriatric health assessments including medical history, diagnosis, treatment review, and health education
- Administer and order point-of-care and lab-based testing as needed
- Provide physician referrals and case management referrals based on assessment findings
- Collaborate with primary care physicians on patient education and follow-up care
- Document assessments clearly according to Matrix guidelines and protocols
- Ensure follow-up and continuity of care
Required Qualifications
- Master's degree or commensurate experience with satisfactory completion of NP licensure
- Current NP licensure in state of practice with prescription authority or ability to obtain prescriptive authority
- Board certification from AANP, AACN, or ANCC in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
- Current BLS, ACLS, or CPR certification
- Valid state driver's license for state of residence
- Automobile insurance meeting the following minimums: $100,000 bodily injury per person/$300,000 per incident, $50,000 property damage, and collision/comprehensive deductibles of $1,000 or less
- Strong computer skills and familiarity with electronic health records and medical record software
- Excellent verbal and written communication skills
Preferred Qualifications
- One year of NP experience (new graduates are encouraged to apply)
- Ability to travel up to 15% of the time, including overnight travel to additional locations as needed
Schedule
Full-time position with typical Monday through Friday schedule, 8:00 AM to 5:30 PM. Assessments are conducted Sunday through Saturday, 8:00 AM to 7:00 PM, but full-time providers typically work the standard Monday-Friday schedule.
Compensation and Incentives
- Annual base salary: $95,500 to $158,400 based on market compensation analysis and experience
- Sign-on bonus: $7,500
- Eligible for full-time clinician incentive plan with monthly (uncapped) and year-end (up to $25,000) incentive opportunities. Average 2025 payout was $1,500 per month and $7,500 at year end, totaling $25,500 annually
