NURSE PRACTITIONER

FellowshipLIFE is seeking a Primary Care Nurse Practitioner to join our medical group on a full-time basis. You will provide comprehensive primary care services to senior residents across our Life Plan Communities in New Jersey, including sub-acute, long-term care, assisted living, and medical office settings. This role involves collaborating with physicians and other healthcare professionals to develop and implement individualized care plans, promote wellness, and manage chronic conditions.

Clinical Setting

You will work three days per week at Applewood Village in Freehold, NJ and one day per week at Pines Village in Whiting, NJ. You will work within a supportive environment alongside well-qualified medical, nursing, and multidisciplinary professionals.

Responsibilities

• Conduct thorough assessments and physical examinations of senior residents
• Diagnose and manage acute and chronic health conditions
• Develop, implement, and monitor individualized care plans
• Prescribe medications and treatments as necessary
• Provide health education and counseling to residents and their families
• Collaborate with physicians, nurses, and other healthcare team members
• Maintain accurate and up-to-date medical records
• Participate in care team meetings and contribute to quality improvement initiatives
• Ensure compliance with all regulatory and organizational policies and procedures

Required Qualifications

• Master's degree in Nursing from an accredited program
• Current Nurse Practitioner license in New Jersey
• Certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP)
• Minimum of 2-3 years of experience in a sub-acute or acute care setting
• Excellent clinical skills and knowledge of geriatric care in post-acute settings
• Strong communication and interpersonal skills
• Ability to work collaboratively with the physician in a team environment
• Patient-centered approach to care
