Nurse Practitioner- Fee for Service

TruHealth, a division of American Health Partners, is seeking an Advanced Practice Registered Nurse (APRN/NP) or Physician Assistant (PA) to provide primary care services to residents in nursing homes and assisted living communities. In this fee-for-service role, you will deliver personalized, coordinated health care to the aging population while working to improve patient outcomes and prevent rehospitalization.

You will exercise independent clinical judgment to assess and treat patients with acute and chronic conditions, consulting with a supervising physician as appropriate. This position offers the opportunity to implement Treat in Place Protocols in compliance with all federal and state regulations while collaborating closely with facility partners and health plan leadership.

Clinical Setting

You will work primarily within nursing homes or assisted living communities, serving Medicare beneficiaries living in long-term care facilities.

Responsibilities

• Assess patients' medical and mental health needs and develop plans of care that proactively manage their conditions and address barriers to care
• Prescribe medications and order lab work, diagnostic procedures, and consultations
• Monitor patients' compliance and response to treatment, modifying care plans as needed
• Ensure compliance with local, state, and federal regulations related to clinical services provided
• Provide training and advice to facility staff as required

Required Qualifications

• Active APRN (Advanced Practice Registered Nurse) or PA (Physician Assistant) license
• Degree from an accredited APRN or PA program
• Three years of clinical experience in nursing or rehabilitation with geriatric populations, or equivalent fee-for-service experience
• Electronic Health Records (EHR) experience
• Working knowledge of Microsoft applications, including Word, Outlook, and Excel

Preferred Qualifications

• Bachelor's degree or higher
• Master's degree or higher
• Primary care or internal medicine experience
• Geriatrics focus
