Nurse Practitioner - Family Medicine Care, Northside

Community Health Network is seeking a Nurse Practitioner with family medicine training to join our outpatient Family Medicine Care, Northside location in Indianapolis. This is a full-time position in a busy family practice office providing primary care services to our patient population.

Clinical Setting

You will work at Family Medicine Care, Northside, located at 7525 E. 82nd Street, Suite A, Indianapolis, IN 46256. This is part of Community Health Network, one of the nation's most integrated healthcare systems with over 1,300 providers across central Indiana. The practice uses a single, robust electronic medical record system (EPIC) to support clinical care and coordination.

Schedule

This position offers flexibility in scheduling:

• 4 days per week at 10 hours per day, or
• 5 days per week at 8 hours per day
• Day shift only
• No weekends or on-call duties

Required Qualifications

• Minimum 3 to 5 years of experience as an Advanced Practice Provider in a primary care setting
• Current national certification from a nationally recognized Advanced Practice Nurse certification examination; must maintain certification on an ongoing basis
• Eligibility for prescriptive authority under Indiana State Board of Nursing criteria; must maintain eligibility on an ongoing basis
