The opportunity

Job Description

EasyHealth is seeking experienced Nurse Practitioners to deliver in-home preventive care to members in Portland, Oregon and surrounding areas. In this role, you will conduct comprehensive preventive health assessments during scheduled home visits, identify care gaps, and connect members to appropriate ongoing care. This position offers autonomous practice in the community with meaningful one-on-one patient interaction, without the traditional clinic environment or back-to-back exam room schedule.

Responsibilities

  • Conduct comprehensive preventive health assessments during in-home visits
  • Identify care needs and close important gaps in member care
  • Connect members to appropriate ongoing care resources
  • Deliver high-quality assessments focused on individual members
  • Travel locally for scheduled in-home visits
  • Work independently while collaborating with the EasyHealth clinical team

Clinical Setting

You will meet members in their homes rather than in a traditional clinic setting. This role focuses on adult and older-adult populations and emphasizes accessibility for members who may face barriers to traditional care.

Required Qualifications

  • Active Nurse Practitioner license in Oregon and/or Washington
  • National board certification
  • Comfortable conducting comprehensive health assessments and clinical decision-making
  • Comfortable traveling locally for scheduled in-home visits
  • Strong assessment and clinical decision-making skills
  • Ability to work independently while being part of a supportive clinical organization

Preferred Qualifications

  • Family Medicine, Primary Care, Internal Medicine, Urgent Care, or Geriatrics clinical experience
  • Licensed in both Oregon and Washington
