Nurse Practitioner

EasyHealth is seeking experienced Nurse Practitioners to deliver in-home preventive care to members in Portland, Oregon and surrounding areas. In this role, you will conduct comprehensive preventive health assessments during scheduled home visits, identify care gaps, and connect members to appropriate ongoing care. This position offers autonomous practice in the community with meaningful one-on-one patient interaction, distinct from traditional clinic settings.

Responsibilities

• Conduct comprehensive preventive health assessments during in-home visits
• Identify care needs and gaps in member health coverage
• Connect members to appropriate ongoing care resources
• Deliver high-quality assessments focused on individual members
• Travel locally for scheduled in-home visits

Clinical Setting

This is community-based in-home preventive care, not a traditional primary care practice or clinic environment. You will work independently while receiving administrative and operational support from the EasyHealth team.

Required Qualifications

• Active Nurse Practitioner license in Oregon and/or Washington
• National board certification
• Ability to conduct comprehensive health assessments
• Strong assessment and clinical decision-making skills
• Comfort working with adult and older-adult populations
• Willingness to travel locally for scheduled in-home visits
• Ability to work independently within a supportive clinical organization

Preferred Qualifications

• Family Medicine, Primary Care, Internal Medicine, Urgent Care, or Geriatrics clinical experience
• Licensure in both Oregon and Washington
