Nurse Practitioner - Eastern Time Zone

Strive Health is seeking a Nurse Practitioner to serve as a Clinical Lead within an interdisciplinary team focused on managing patients with chronic conditions including congestive heart failure (CHF), chronic kidney disease (CKD), end-stage kidney disease (ESKD), and other complex chronic illnesses. This role emphasizes longitudinal, patient-centered care delivered across multiple settings including patient homes, telehealth, clinics, and dialysis centers. You will manage a defined patient panel with accountability for quality, cost, and outcomes while leveraging advanced analytics and technology platforms to reduce administrative burden and maximize time for meaningful clinical care.

Clinical Setting

This is a hybrid-remote position with flexibility to work from home while fulfilling in-person needs at the office, clinic, or during patient home visits. You will serve patients across multiple care settings including patient homes, telehealth visits, and dialysis clinics or partner medical offices where applicable. Business travel may be required to connect with stakeholders, serve patients, and attend company-sponsored team events.

Responsibilities

• Manage a defined patient panel with accountability for quality, cost, and outcomes
• Collaborate with external providers and care teams to ensure alignment of goals, treatment, and care plans
• Obtain patient history, perform physical examinations, order and interpret diagnostic tests, and formulate plans for individual patient short-term and longitudinal needs
• Ensure assessment and care plans incorporate best practices for chronic kidney care, including management of all CKD stages, ESKD, and heart failure, and support transitions to renal replacement therapy, transplant, or conservative care
• Participate in shared decision-making and end-of-life and advanced care planning discussions
• Accurately document and capture diagnoses to support risk adjustment (HCC coding) and identify and address care gaps related to HEDIS and preventative care
• Present patient cases and provide clinical support during clinical rounds and interdisciplinary team meetings
• Perform patient visits in multiple care settings including patient homes, telehealth, and dialysis clinics
• Maintain current board certification and state-specific continuing education requirements
• Adhere to Strive Health's documentation policy
• Provide in-person patient care which may include standing, sitting, walking, pushing, pulling, and lifting
• Meet in person with internal and external stakeholders to facilitate team and business priorities

Required Qualifications

• Master's degree in Nursing or similar field
• 2 or more years of experience as a Family Nurse Practitioner (NP), Primary Care Nurse Practitioner (NP), or equivalent with specialization in cardiology (CHF) or nephrology (CKD)
• 2 or more years of experience with Electronic Health Records (EHR)
• Current state-specific licensure
• National Board Certification
• Current Drug Enforcement Administration (DEA) license or eligible to obtain within 90 days of hire
• Current BLS or CPR Certification
• Efficient and reliable transportation, including an active driver's license, allowing for travel across an assigned region to meet patient needs
• Basic computer and Microsoft Office skills
• Internet connectivity with minimum speeds of 3.8 Mbps download and 3.0 Mbps upload with latency less than 60 ms

Preferred Qualifications

• Demonstrated expertise in CKD management (Stages 3-5), ESRD care, and dialysis management (hemodialysis and/or peritoneal dialysis, home hemodialysis), including medication optimization, volume assessment, and complication management
• Experience leading care transitions across settings (hospital to outpatient dialysis) with a focus on reducing readmissions and ensuring continuity of care
• Proven ability to deliver comprehensive patient education on kidney disease progression, dialysis modality selection (home versus in-center), and kidney transplant pathways to support informed, shared decision-making
• Wound Care certification
• Experience using audio-visual technology platforms

Compensation and Benefits

Annual Salary Range: $112,000 to $140,000. This position is also eligible for a target annual bonus of 10%. Final compensation will be determined based on location, experience, and qualifications.
