Nurse Practitioner

CleanSlate Centers is seeking a Nurse Practitioner to provide medication-assisted treatment (MAT) for patients with opioid and alcohol use disorders. You will work as part of a clinical team delivering evidence-based care in an addiction medicine setting, with a focus on patient-centered treatment and recovery support.

Responsibilities

• Provide medication-assisted treatment using buprenorphine, Vivitrol, Sublocade, and Brixadi
• Evaluate patients and determine individualized treatment plans
• Monitor patient progress and adjust medications as needed
• Collaborate with physicians and behavioral health professionals on complex cases
• Maintain accurate documentation in the electronic medical record within 48 hours
• Participate in clinical meetings, training, and quality improvement efforts
• Support telemedicine and occasional regional coverage as needed

Required Qualifications

• Active Nurse Practitioner license in state of employment
• DEA registration in state of employment
• Graduate of an accredited Nurse Practitioner program
• Strong interpersonal and critical thinking skills
• Comfort with electronic medical records and fast-paced clinical settings

Preferred Qualifications

• Experience in addiction medicine or behavioral health

Schedule

• Monday through Friday
• No weekends, nights, or on-call duties
