American Physiatry is seeking a Nurse Practitioner to join a field-based team providing care coordination and chronic disease management to residents in skilled nursing facilities throughout Maryland. In this role, you will travel statewide to meet with medically complex older adults, work closely with patients and families, and collaborate with facility staff and healthcare providers to improve outcomes and reduce preventable hospitalizations.
Clinical Setting
You will work in skilled nursing facilities across Maryland, serving residents with multiple chronic medical conditions. This is an autonomous, field-based position that requires extensive travel throughout the state.
Responsibilities
- Travel to skilled nursing facilities throughout Maryland to conduct patient visits and assessments
- Identify residents who may benefit from additional care coordination and ongoing support
- Educate patients and families regarding chronic conditions, medications, and treatment recommendations
- Develop and maintain individualized care plans for medically complex patients
- Collaborate with facility staff, primary care providers, specialists, and other members of the healthcare team
- Support residents during transitions of care and help reduce avoidable hospitalizations and emergency department visits
- Complete accurate and timely documentation in the electronic medical record
- Build and maintain strong relationships with facility leadership and interdisciplinary teams
Required Qualifications
- Active Maryland Nurse Practitioner license
- Board certification as an FNP, AGNP, Adult-Gerontology Primary Care NP, or Adult-Gerontology Acute Care NP
- Valid driver's license and reliable transportation
- Ability to work independently and manage a flexible schedule
- Strong organizational and documentation skills
- Excellent communication and interpersonal skills
Preferred Qualifications
- Experience in skilled nursing, long-term care, geriatrics, primary care, home health, hospice, or chronic disease management
Travel Requirements
- Willingness to travel extensively throughout Maryland
- Occasional overnight travel may be required depending on facility locations
