Chestnut Health, a leading multi-specialty healthcare organization, is seeking a Nurse Practitioner to join our Physical Medicine and Rehabilitation (PM&R) team. In this role, you will work in a collaborative, patient-centered environment focused on restoring function and improving quality of life for individuals with injuries, illnesses, or disabilities. Clinical Setting
You will practice within our physiatry specialty, collaborating with therapists, nurses, and other healthcare professionals in a multidisciplinary team environment.
Responsibilities
- Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs
- Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations to determine appropriate rehabilitation goals and interventions
- Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence
- Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills
- Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems
- Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care
Required Qualifications
- Active state license to practice as a Nurse Practitioner
- Current Controlled Substance License (required upon start date)
- Current DEA License (required upon start date)
- ANCC Certificate or AANP Certificate
- Certified Nurse Practitioner credential
- Desire to work in Physiatry/Physical Medicine and Rehabilitation
