Capital Cardiology Associates is seeking a full-time Nurse Practitioner or Physician Assistant to provide comprehensive cardiometabolic care in our clinic. In this specialized role, you will assess patients and develop evidence-based treatment plans in collaboration with a multidisciplinary team including cardiologists, pharmacists, dietitians, exercise trainers, nurses, and health coaches. You will manage patients across cardiovascular, endocrine, weight management, kidney disease, and lifestyle medicine components.
Clinical Setting
This position is based in our cardiology clinic and operates in person. You will work as part of an integrated team focused on delivering patient-centered cardiometabolic care.
Responsibilities
Patient Care Management
- Conduct thorough assessments of patients to evaluate cardiovascular, kidney, and metabolic risks, including medical history review, physical exams, and ordering appropriate diagnostic tests
- Order and interpret diagnostic and therapeutic tests relative to patient needs
- Prescribe appropriate pharmacological and non-pharmacological treatment modalities
- Develop personalized, evidence-based management plans considering medical history, current health status, and individual patient goals
- Assess patients for cardiovascular risk, metabolic and endocrine conditions, diabetes, kidney disease, sleep apnea, and obesity
- Evaluate patients' readiness for change and engage them in shared decision-making
- Educate patients about pharmacological cardiometabolic options, including anti-obesity medications, risks and benefits, and lifestyle changes
- Provide patient counseling and motivational interviewing techniques
- Monitor patients' progress, assess and manage complications, and adjust treatment plans as needed
- Monitor the effectiveness of interventions and provide ongoing support
- Collaborate with the multidisciplinary team to incorporate lifestyle medicine and achieve optimal patient outcomes
- Seek consultation from supervising physician when appropriate
- Assist other clinical areas during downtime, including insurance appeal clinical justification
Clinical Documentation and Communication
- Maintain clear, organized, up-to-date, and accurate clinical notes that demonstrate understanding of documentation requirements for the level of visit performed
- Work closely with cardiologists, pharmacists, dietitians, medication access specialists, and other healthcare professionals to create a multidisciplinary approach to patient care
- Effectively communicate clinical findings and plan of care to physicians, patients, families, and other members of the healthcare team

- Function independently within scope of practice as delineated by New York State
- Maintain required certifications and licensures
- Promote a positive public image for the profession and for Capital Cardiology Associates

- Assist with training and education of staff and contribute to an open learning environment
- Demonstrate flexibility in work location to accommodate organizational needs
- Contribute to quality improvement initiatives by collecting and analyzing data, participating in case reviews, and suggesting process enhancements
- Stay current with advancements in cardio-kidney-metabolic and obesity guidelines and research to offer evidence-based care
Required Qualifications
Education
- Master's degree from an accredited Nurse Practitioner or Physician Assistant program
Licenses and Certifications

- BLS (Basic Life Support) certification
- ACLS (Advanced Cardiovascular Life Support) certification
Preferred Qualifications

Compensation and Benefits
Salary: $107,000 to $120,000 annually, based on experience and qualifications
